These are strict requirements:
Your EX-A document should refer clearly to you (not your business) and your items. Please make sure the following information is present on your export document:
1. Your personal name (the same as on your purchase receipt)
2. The name of the store
3. The invoice / receipt number from the store
4. Your items purchase value
